Ad hoc expert meeting Recommendation if type of oral formulation (IR/MR) unknown? Evidence in Sweden clearly showed significant problems when market share of MR product was high. In overdose, patients frequently not able to inform exactly on product/ agent ingested. Not knowing which formulation been taken, potentially imply patients need to be treated as if they took MR formulation. Would make results of a single s paracetamol blood test uninterpretable; need for repeated s paracetamol measurements; increase hospital admissions unnecessarily in many instances. 17 Ad hoc expert meeting Is there data to support that a modified treatment nomogram / treatment recommendations would reduce risk for hepatic injury to level seen for IR paracetamol? Experts noted that currently available evidence is limited; modifications to treatment protocols generally based on low grade evidence. Relatively low rate of severe liver failure with IR products following NAC treatment, and need for systematic collection of biomarker data would make it difficult to detect differences in effectiveness of treatment strategies with MR products. SE overall conclusion Increased uncertainty in RMM leads to increased risk for serious hepatic injury Unacceptable also with occasional additional serious outcomes due to these uncertainties These uncertainties do not outweigh benefit in indications for symptomatic relief of e.g. headache, common cold, toothache, and fever, or (very) modest efficacy in longer term pain conditions Benefit Risk balance unfavorable 23 Råd till sjukvården (Info från LV januari 2017) Det är viktigt for sjukvården att uppmärksamma att förgiftningar med Alvedon 665 mg kräver förändrade behandlingsprinciper och bör handläggas i samråd med GIC. Den omfattande förskrivningen innebär att risken for dessa förgiftningar ökat de senaste åren. Flertalet paracetamolförgiftningar är medvetna impulshandlingar i självskadesyfte. Det finns därför anledning att särskilt noga försöka anpassa förskriven mängd till patientens förväntade behov. Alvedon 665 mg marknadsförs nu också i en mindre förpackning innehållande 48 tabletter.
